Scheduling and forms

Online appointment scheduling is provided by Acuity Scheduling. When you follow a scheduling link, the information you submit is handled through Acuity and according to the privacy and account settings that apply to our scheduling service. The website may record that a scheduling link was selected, while Acuity may report that a booking was completed.

Contacting us

If you call or email us, we use the information you provide to respond to your question or appointment request. Please do not send sensitive medical information by ordinary email.
